Diagnosis of premature ejaculation

It is typical for men to feel at least some control over ejaculation during patterned sexual intercourse. If you feel so, you should consult a medical expert. Premature ejaculation may be present.

If premature ejaculation gets in the way of your sexual experience, it's time to seek medical help. The diagnosis is based on whether you ejaculate early, late, or not. Your doctor will conduct a physical examination. They will talk to you about your sex life and relationship with your partner. You might be asked to run through some blood tests to check your testosterone levels.

The doctor might ask you questions like:

  • How long have you had this?
  • How often does it happen?
  • Does it happen at every attempt?
  • When did you first experience it?
  • How is your interpersonal relationship?
  • Is there anything like drugs or alcohol that makes premature ejaculation better or worse?
  • Do you use any recreational drugs?
  • Are you on any medication?
  • How much premature ejaculation bothers you?

Treatments for premature ejaculation

Standard treatment options for premature ejaculation include behavioral techniques, topical anesthetics, medications, and counseling. Behavioral techniques

The pause-squeeze technique

Your doctor may instruct you and your partner on the pause-squeeze technique. This method works as follows:

  1. Start sexual activity as usual, including stimulating the penis until you feel almost ready to ejaculate.
  2. Your partner can squeeze the glans of your penis, where the head joins the shaft, and maintain the squeeze for several seconds until your urge to ejaculate passes.
  3. Your partner can repeat the squeeze process as required.

This helps you in delaying the ejaculation. After some practice sessions, you will be habitual in delaying the ejaculation, and you won't require the pause-squeeze technique.

Start-stop technique

If the above-mentioned pause-squeeze technique causes pain or discomfort, another technique is to stop your sexual stimulation just before ejaculation, talk about something else with your partner, wait until the level of arousal diminishes, and begin again. This technique is known as the stop-start technique.

Condoms

Condoms can decrease your penis sensitivity, which can help in delaying ejaculation. "Climax control" condoms are available in the market over the counter. These condoms contain anesthetic agents such as benzocaine or lidocaine or are made of thicker latex to delay your ejaculation.

Medication for premature ejaculation

There are several anesthetic creams and sprays that contain a numbing agent, like benzocaine, lidocaine, or prilocaine, which are sometimes used to treat premature ejaculation. These can be applied to the penis 10 to 15 minutes before sex to reduce sensation and help delay ejaculation.

A lidocaine-prilocaine cream for premature ejaculation (EMLA) and lidocaine sprays for premature ejaculation are available by prescription and over-the-counter.

Oral medications for premature ejaculation

These medications include antidepressants, analgesics, and phosphodiesterase-5 inhibitors. These medications are prescribed for either on-demand or daily use, alone, or with other treatments.

  • Antidepressants

A side effect of some antidepressants is delayed orgasm. That's why selective serotonin reuptake inhibitors (SSRIs) such as sertraline (Zoloft), escitalopram (Lexapro), paroxetine (Paxil), or fluoxetine (Prozac, Sarafem) are used in the treatment of premature ejaculation.

It takes five to ten days for these medications to work. But it reaches its full effect after two to three weeks of treatment.

If SSRIs don't improve your premature ejaculation, your doctor might prescribe the tricyclic antidepressant clomipramine (Anafranil). It has other unwanted side effects such as nausea, perspiration, drowsiness, and decreased libido.

  • Analgesics 

Tramadol is a medication used to treat pain. It also has side effects that delay ejaculation. Its unwanted side effects might include nausea, headache, sleepiness, and dizziness.

Tramadol is not used in combination with an SSRI

  • Phosphodiesterase-5 inhibitors.

Some medications used to treat erectile dysfunction, such as , tadalafil (Cialis, Adcirca), sildenafil (Viagra) or vardenafil (Levitra, Staxyn), also might help premature ejaculation. The potential unwanted side effects include headache, facial flushing, and indigestion. These medications might be more effective when used in combination with an SSRI.
